How to Apply for Vidhwa Pension Uttar Pradesh?

If a widow woman wants to apply for Uttar Pradesh Widow Pension Scheme 2021, then follow the steps given below or go to the link.

Visit Official Site

First of all go to the official website of Widow Applicant Women Integrated Social Pension Schemes. Where the home page of the official website will open.

Apply Online

The option of Apply Online will appear with the icon. Click on the option of “Apply Online”. A new page will open on your screen.

Four Options

You will see four options (i) New Entry Form (ii) Edit Saved From/Final Submit (iii) View Application Form (iv) User Manual.

Registration Form

Click on the option of New Entry From. The format of application for destitute women's pension will open in front of you.

Personal Details

In the Personal Details section, select District, Tehsil, Resident, Category and Sub Category (District, Tehsil, Village Category & Sub Category) from the Droplow Box.

Upload Photo

Fill the details of applicant's name, husband's H.No., locality, street, pincode and full address etc. Upload color passport size photograph (JPEG max size 20KB).

Upload Birth Certificate

Fill your date of birth and age and upload the education record or medical officer's age certificate (PDF format & size less than 500KB).

Mobile/Account Details

Fill mobile number & in the Bank Details section, select the name of the bank and  bank branch from the drop down box and enter the account number.

Upload Bank Passbook

Upload copy of bank account passbook with IFSC Code, upload documents should be in PDF format and its size should not exceed 500KB.

Upload Income Certificate

In the income statement, fill the total annual income of the family and the income certificate number provided by the Tehsildar & upload the same income certificate.

Husband Details

In the Other Important Details section, fill the date of death of the applicant's husband in the relevant column and upload the death certificate.

Self Declaration

Finally, select Self Declaration and fill in the Captcha Code. After this, after verifying the complete details, click on the button of Save.

Acknowledgment Slip

Your Registration Acknowledgment Slip will be ready as soon as you save it. Keep in mind, print out the Acknowledgment Receipt to see the status.

Physically Documentation

Physically submit to DSWO/DPO/DHWO office within 1 month from the date of submission of application along with copy of uploaded  documents.

Acknowledgment

Acknowledgment must be taken after physically submitting the copy of the application form and documents to the DSWO/DPO/DHWO office.
